And he is the only one.

Prior to the lottery teams could be rewarded (much moreso) by tanking.  Worst record got 1st pick.  Ties were broken by coinflip.

Tim Duncan was a once-a-decade prospect who went to a team who happened to have one of the three best centers in the league. That was a perfect storm. Probably won't happen again.
